Form 1095-C will now be provided electronically, in line with the Affordable Care Act (ACA) and the Paperwork Reduction Act. This means you will no longer automatically receive a paper copy by mail. To access your Form 1095-C online, please visit:

Design Services - Healthcare Facility
Granville Vance Public Health (GVPH) is inviting qualified design firms to submit Statements of Qualifications (SOQ) to provide on-call design consultation services for healthcare facilities across our two-county public health district.
GVPH serves Granville and Vance counties in North Carolina and provides a wide range of clinical, preventive, environmental, dental, WIC, and population-health services across six facilities. As our buildings age and service demands evolve, we are seeking a design partner to support ongoing space improvements that enhance patient experience, operational efficiency, and workforce functionality - while centering trauma-informed design principles.
At this stage of our organizational evolution, GVPH is seeking an experienced healthcare design firm to provide as-needed consultation and full-scope design services for renovations and improvements in clinical, lobby, and program spaces.
Purpose of This RFQ
The purpose of this Request for Qualifications (RFQ) is to identify highly qualified design firms with demonstrated expertise in healthcare facility design and trauma-informed environments.
This RFQ is issued in accordance with North Carolina’s Mini-Brooks Act (N.C.G.S. § 143-64.31), which requires selection based on demonstrated competence and qualifications without regard to fee at this stage of the process.
The selected firm(s) will support GVPH in planning and implementing design improvements across multiple facilities, including:
- Environmental health offices
- Patient registration and lobby areas
- Diagnostic testing and support services
- Design solutions must incorporate trauma-informed principles that promote safety, dignity, resilience, confidentiality, and healing - particularly for vulnerable and trauma-affected populations.
Scope and Engagement Approach
The selected firm will provide comprehensive design services, which may include:
- Program validation and space planning
- Schematic design through construction documents
- Code analysis and permitting
- Coordination with GVPH, consultants, and regulatory authorities
- Construction administration
- Integration of trauma-informed design strategies into layout, material selection, and patient/clinician circulation
Engagements will be structured as on-call services and may be activated as needs arise across our Granville and Vance County facilities. Projects may vary in size and complexity, and will be scoped collaboratively based on agency priorities and available funding.

- Familiarity with federal and state grant compliance requirements
- Experience navigating regulatory and healthcare environments

Minimum Qualifications
To be considered, responding firms must demonstrate:
- Licensure and registration to practice in North Carolina
- Successful completion of healthcare facility projects of similar scope and complexity
- Specific experience or training in trauma-informed design
- Project teams with healthcare and trauma-informed design expertise
- Experience collaborating with healthcare providers and regulatory authorities
- Familiarity with federal and state grant compliance requirements, including Uniform Guidance (2 C.F.R. Part 200) and applicable North Carolina statutes
Evaluation and Selection
Statements of Qualifications will be evaluated by a GVPH selection committee based on:
- Firm experience, reputation, and workload capacity
- Project management approach and grant administration experience
- Experience and qualifications of the proposed project team
- Thoroughness and clarity of the submission
- In accordance with 2 C.F.R. 200.318 and other applicable procurement standards, contracts may only be awarded to responsible firms demonstrating integrity, compliance with public policy, past performance, and adequate financial and technical resources.
- Interviews may be conducted with top-ranked firms. Fee proposals will be requested from the most qualified firm(s) during subsequent negotiations.

Granville Vance Public Health (GVPH) is inviting qualified operations consultants and consulting firms to submit Statements of Qualifications to support ongoing organizational effectiveness, systems alignment, and governance development across our two-county public health district.
GVPH serves Granville and Vance counties in North Carolina and provides a wide range of clinical, preventive, environmental, and population-health services. Like many public health agencies, we are navigating an evolving landscape shaped by expanding service models, increasingly complex funding streams, workforce sustainability challenges, and growing expectations for accountability, performance, and impact.
At this stage of our organizational evolution, GVPH is seeking an experienced operations partner to support discrete, as-needed projects focused on strengthening internal systems while preserving the mission-driven, relationship-centered culture that defines our work.
Purpose of This RFQ
The purpose of this Request for Qualifications (RFQ) is to identify consulting partners with expertise in public health or healthcare operations, process improvement, people-management systems, and governance design. The selected consultant(s) will work collaboratively with GVPH leadership and staff to assess current operations, identify opportunities for improvement, and support implementation strategies that enhance efficiency, clarity, and sustainability.
This work is also intended to position GVPH for expanded system leadership roles, including cross-sector coordination and potential participation as a Rural Health Transformation Grant Hub. As such, strong experience in governance development, decision-making frameworks, and multi-partner initiative readiness is especially important.
Scope and Engagement Approach
Consulting engagements may include organizational and workflow assessments, evaluation of people-management and HRIS systems, governance and accountability structure design, change-management support, and implementation planning. Projects will be scoped collaboratively and may occur over time, based on agency priorities and capacity.
GVPH values practical, implementable recommendations; strong facilitation and stakeholder engagement; and approaches that center staff experience, workforce sustainability, and continuity of services for the communities we serve.

Request for Proposals: Region 7 Lead Data Analyst for Performance Measures Study
Pursuant 45 CFR § 74.44 and 2 CFR § 200.320 , Granville-Vance District Health Department is issuing this request for proposals to qualified responders to manage a Lead Data Analyst contract to develop and implement performance measures for local health departments (LHDs) until May 31, 2023.
Bidders must be a staffing agency with a strong background in public health work in North Carolina as well as experience undertaking complex issues. The selected firm must have a proven track record of filling critical gaps in public health agencies by providing personnel services, directly or indirectly, to analyze performance measures to ensure LHDs are accurately reporting core shared performance measures. The firm must have experience working with LHDs and a demonstrated ability to provide high quality services on time and within budget.
The successful bidder will manage the contracts of a Lead Data Analyst contracted to to develop and implement performance measures for LHDs, including Region 7 LHDs. The procurement will be awarded on a fixed-price basis, and bidders should submit the fixed price within their proposal. The project and services will be executed in a manner that does not conflict with the allowable expenses set forth by the federal grant. All services must be completed by May 31, 2023.

Pursuant 45 CFR § 74.44 and 2 CFR § 200.320 Appalachian District Health Department dba AppHealthCare, Cumberland County Health Department, Transylvania County Health Department, Cabarrus Health Alliance, Granville-Vance District Health Department, Albemarle Regional Health Services, Foothills Health District, are issuing this request for proposals to contract a staffing agency to manage two positions that will review and conduct an ongoing analysis of the Medicaid Separate Direct Payments to the local health departments (LHDs) until May 31, 2023.
Bidders must be a staffing agency with a strong background in public health work in North Carolina as well as experience undertaking complex issues. The agency must have a proven track record of filling critical gaps in public health agencies by providing personnel to analyze revenue, some of which Covid-related, to ensure local health departments are maximizing revenue to continue operations. The agency must have experience working with local health departments. The staffing agency must also be able to bill each health department directly for a portion of the fixed cost of the contract.
The successful bidder will manage the contracts of two staff members hired to review and conduct an ongoing analysis of the Medicaid Separate Direct Payments to LHDs, including Region 7 LHDs. The procurement will be awarded on a fixed-price basis, and bidders should submit the fixed price for each region’s health department. The project and services will be executed in a manner that does not conflict with the allowable expenses set forth by the federal grant. All services must be completed by May 31, 2023.

The Teen PEP program is an interactive class where you receive snacks weekly, medically accurate sexual health information, opportunities for discussion amongst your peers, skill development, and fun!! Teen PEP is an elective that takes place at VCHS during 4th block!
In the beginning of the class, Teen PEP students will vote on and receive one FREE Teen PEP apparral to wear when representing Teen PEP! Students will also vote on two gifts that they want to work towards earning throughout the class (ex: Apple iPad/Airpods). Then at the end of the class, whichever two students that had the best attendance and/or participation will receive those gifts that we previously selected!! Everyone else who passes the class with a grade of C or better will receive a $50 Visa gift card!

“On Monday, December 27, 2021, in response to the rapid spread of a new Omicron variant of COVID-19, the Centers of Disease Control and Prevention (CDC) released a media statement updating and shortening the recommended isolation and quarantine period for the general population.
GVPH is adjusting local quarantine and isolation guidance in the interim, based on the latest updates from and NC DHHS in response to Omicron’s spread.”

Update from Local Public Health for School Safety Fall 2021
In keeping with CDC guidance and good infection control practice, Granville Vance Public Health recommends universal indoor masking for all teachers, staff, students, and visitors to K-12 schools, regardless of vaccination status. Children should return to full-time in-person learning in the fall with layered prevention strategies in place.
Earlier today, a notification was shared with all schools and child care centers in the district inviting nurses and leaders to the next webinar hosted by Dr. Shauna Guthrie, our GVPH medical director. This School/Child Care COVID-19 Check-in will be held on Friday, August 6th, at 3 pm. This will be a chance to learn the latest CDC and NCDHHS guidance/recommendation for schools, ask Dr. Guthrie any questions you may have, and meet Jesus Peralta, our new GVPH COVID-19 Liaison for School/Child Care centers.

Vegetable and Fruit Increase Provides More Nutritional Access to WIC Families
June 21, 2021
On March 12, 2021, the American Rescue Plan Act authorized State WIC Agencies to enhance the WIC benefit for a period of four months, expanding the vegetable and fruit voucher from $9/month for children and $11/month for women to $35/month per participant. WIC’s vegetable and fruit voucher – known as the Cash Value Benefit (CVB) – was first introduced in 2007 as part of a decades-long effort by the National WIC Association (NWA) to increase the nutritional quality of WIC-approved foods. The CVB is credited with improving the dietary quality of WIC participants and reducing the prevalence of childhood obesity among WIC toddlers.
North Carolina WIC will be implementing the CVB increase beginning June 1, 2021 until September 30, 2021.

Feb. 19th Vaccine Clinic Update

Granville Vance Public Health will be opening at 10:30am today, Friday, February 19th.
We are still planning to host our vaccine clinics today. The second dose clinic at Granville County Expo Center will begin at 12pm and all morning appointments have been rescheduled to the afternoon. Those who have second dose appointments scheduled this morning in Vance County should arrive anytime between 10:30am-4:30pm to receive your shot.
Granville Vance Public Health has rescheduled all vaccine clinics originally planned for February 18th. Those who had appointments at the Vance County Health Department to receive their second dose have been rescheduled for February 25th at the same time and place as your original appointment. Those who had appointments for their first dose at Ritchie Brothers in Butner, may call our hotline at 252-295-1503 on Monday, February 20th to reschedule your appointment if you have not already been rescheduled.

2nd Dose Clinic on Feb. 14th POSTPONED to Feb. 21st

The Second Dose Vaccine Clinic at Clarke Elementary School and Vance County High School scheduled in Henderson, NC, for February 14th has been postponed until the same time on Sunday, February 21st.
Winter weather has caused organizers to make a decision considering driver safety. Again, we will host the second dose Pfizer vaccine clinic in Henderson, NC, at Clarke Elementary School and Vance County High School on Sunday, February 21st from 10am until 4pm.
If you attended our first dose vaccine clinic on January 24th, you do not need to make an appointment to come on Sunday, February 21st. We have your second dose ready and you are already registered for this clinic.
Only those who were vaccinated with a first dose of Pfizer vaccine on January 24th will receive vaccine on February 21st.
There is a window of up to six weeks past the first dose to receive your second dose that is an acceptable recommended timeframe and follows administration guidance from the vaccine manufacturer. There is no harm in waiting one more week to ensure everyone’s safety on the roads.

GVPH Vaccinating 1a, Soon to Move to 1b
January 7, 2021
Granville and Vance Counties are currently in the Phase 1a distribution and we are giving vaccines every day. We plan to move to Phase 1b, Group 1 (only those aged 75 and over), on Thursday, January 14th as we continue prioritizing those at highest risk for severe illness and death due to COVID-19. It will take us through the month of March to make good progress in all the groups within these first two phases (1a and 1b). We expect to be move from phase 1b into other phases in the spring.
For this initial phase, 1a, we are reaching out to eligible health care organizations and individuals with additional instructions to sign up for vaccination. Please wait until the phase you are eligible for is announced on the radio, the newspaper, or on our GVPH COVID-19 Vaccine website at https://www.gvph.org/covidvaccines/ to reach out to the health department about making an appointment for the vaccine. We will provide more information the week of January 11th about how to register in the mandatory statewide COVID Vaccine Management System (CVMS). There is a survey everyone must fill out as part of registration and the system is not accessible to individuals at this time.
For individuals in Phase 1b, Group 1 (only individuals who are 75 and older), beginning the week of January 11th, they can call the health department to register and schedule their appointment. Any other instructions that may help make your vaccine appointment move faster (such as forms you need to fill out ahead of your appointment) will be posted on our website. If you are not in Phase 1a or Phase 1b Group 1, please refrain from contacting the health department until we announce we are moving into later groups and phases. Given the volume of calls we are receiving, a Granville Vance COVID Vaccine Information Hotline has been established at 252-295-1503.

GVPH Administers First Doses of COVID-19 Vaccine!

Granville Vance Public Health is thrilled to announce a momentous occasion in our fight against the COVID-19 pandemic – we received of our initial shipment of Moderna vaccine doses in Granville on Monday, December 21st. We received a total of 700 doses across our Granville and Vance locations combined. On Tuesday, December 22nd, we administered our first few doses to some of our clinical staff at the health department who qualify for Phase 1A. Our team is taking inventory, preparing the ancillary supplies, training on computer systems and vaccine handling, and preparing to vaccine others. This hallmark in the pandemic experience marks the beginning of the end! So much good public health work lies ahead.

“Although the number of active cases in our counties had been trending downward over the past few months, our data have quickly begun trending in the wrong direction again. Unfortunately, we have set numerous records this week for number of new cases and deaths in North Carolina and the United States. As of today, November 23rd, we have 255 active cases of COVID-19 in Granville County and have recorded 56 deaths over the course of the pandemic response. In Vance County, we have 414 active COVID-19 cases and 54 deaths…
Please remember – every action you take to reduce the spread of COVID-19 in a given setting, whether at the grocery store, in your workplace, or at a family gathering, adds up to make it harder for the virus to transmit from person to person. And, the more we can reduce the spread, the more lives we can save. We want to gather again for the holidays next year, so let’s do all we can to preserve our health this year.”

WIC Waivers Have Been Extended Until Further Notice

USDA’s Food and Nutrition Service has extended certain active waivers to provide flexibilities as state agencies provide WIC services while working towards reopening.

UnitedHealthcare Donates $125,000 to Granville-Vance Public Health

UnitedHealthcare has awarded a $125,000 Empowering Health grant to Granville-Vance Public Health to help us assist Granville County Public Schools with distributing meals to homebound students so they don’t go hungry when school is out.
In total, UnitedHealthcare is donating $12.3 million through Empowering Health grants across 21 states to expand access to care and address the social determinants of health for uninsured individuals and underserved communities.
We’re grateful for UnitedHealthcare’s support as we work to further our mission of protecting and promoting health in Granville and Vance Counties. It’s important now, more than ever, that we work together.
GVPH Health Director Lisa Macon Harrison Elected Board Vice President of the National Association of County and City Health Officials

The National Association of County and City Health Officials (NACCHO) has elected Lisa Macon Harrison, Local Health Director of Granville Vance Public Health, to its 2020-2021 Board of Directors as Vice President. NACCHO is the voice of the nation’s nearly 3,000 local governmental health departments.
Ms. Harrison assumed the Vice President role on July1, 2020. She will then serve as president-elect in 2021-2022 and president in 2022-2023.

GVPH Carolina Fellows Family Dentistry receives grant award from The Duke Endowment

We received some wonderful news this week! Carolina Fellows Family Dentistry, our dental clinic, received a $400,000 grant from The Duke Endowment to expand dental care in rural and underserved schools in Granville and Vance Counties. The grant enables Carolina Fellows Family Dentistry to launch school-based programs that prioritize preventive services (such as regular cleanings) and address certain restorative treatments (including fillings) for school-aged children. COVID-19 Press Release April 13th

As of April 13, 2020, there are 95 known confirmed cases of COVID-19 in Granville County. Sixty-six of the 95 cases in Granville County are inmates at the Federal Bureau of Prisons Complex in Butner, NC, while the other 29 are community-based cases. Vance County has 21 confirmed active cases of COVID-19. Sadly, we have had our first reported deaths due to complications from COVID-19 in both counties.

The Granville Vance WIC program has transitioned all WIC appointments to over the phone contacts effective March 26, 2020 until further notice. WIC staff will call you one business day before your scheduled appointment and complete the appointment over the phone.
We are continuing to serve NEW WIC families that are pregnant, postpartum, or a child under the age of five.
If you do not have a scheduled WIC appointment, call either the Granville County WIC office at (919) 693-2141 or Vance County WIC office at (919) 693-2141 and an appointment will be made for a phone contact.
COVID-19 Update to GVPH Clinic Operations

Due to the current COVID-19 pandemic, our office policies and restrictions will be updated frequently. We are sorry for any inconvenience this may cause any of our patients.
We are doing our best to minimize risks of communicating COVID-19. After careful consideration and guidance from the CDC, NCDHHS and the WHO we will be enacting the following office policies effective March 23,2020:
- If you have any flu-like symptoms (fever, cough, shortness of breath or other respiratory symptoms) or you are concerned that you may have been exposed to COVID-19 please do not come to our office. Please first call our office at (919) 693-2141 or (919) 693-2141 and we can guide you to next steps regarding testing and where to be seen.
- We are strongly discouraging visitors during your visits. We understand that with many schools released this may not be possible and we will make exceptions on a case by case basis.
- We will be rescheduling routine annual visits and we encourage you to call and cancel your visits for non-urgent medical needs. We may be able to refill prescriptions as needed when we speak with you.
- We will be moving some visits to phone and televisits (video). This is a work in progress. We will be contacting you if this is an option for your visit.
- We will likely adopt a protocol for less frequent prenatal visits suggested by the World Health Organization. We will call you if we plan on rescheduling your appointment. Please rest assured that we will not reschedule your visit if we feel like this is, in any way, unsafe for you or your baby.
Please know that we are doing our very best to ensure the safety and well-being of our patients and their families. Thank you for understanding.
